When Should You Send an Appointment Reminder Text? 3 Days and 1 Day Before
By Sharrk Team · October 26, 2026
Send appointment reminder texts twice: once about 3 days before the appointment and again 1 day before. Sharrk handles the day-before reminder for you: every appointment booked on the Sharrk calendar gets a confirmation email and text right away, then a reminder email and text 24 hours before by default.
That two-touch schedule comes from the largest randomized trial we found that tested reminder timing directly. Below are the schedule, sample texts for estimate visits, service calls and med spa appointments, what the research does and doesn't show, and the consent basics.
When should you send an appointment reminder text?
About 3 days before, and again 1 day before. If you only send one, send it the day before.
The best evidence on timing is a 2018 randomized trial published in The American Journal of Managed Care by John F. Steiner and colleagues at Kaiser Permanente Colorado. They randomized 54,066 primary care visits at 25 clinics into three groups, with reminders sent by automated text or voice call:
| Reminder schedule | Missed appointment rate | Missed rate, highest-risk patients |
|---|---|---|
| One reminder, 3 days before | 5.8% | 25.0% |
| One reminder, 1 day before | 5.3% | 24.2% |
| Two reminders, 3 days and 1 day before | 4.4% | 20.5% |
The authors concluded: "Two automated reminders were more effective than 1 in reducing missed appointments and did not reduce visit satisfaction." Cancellation rates were not affected either. So the second text didn't annoy people into canceling. It just got more of them to show up.
Two honest caveats. This was a health system, not a plumbing company or a med spa, and the reminders were a mix of texts and automated calls. Almost all published no-show research comes from healthcare. Still, the reason people miss appointments is similar everywhere: they forget, or plans change and nobody tells you. An early reminder gives them time to reschedule. A day-before reminder catches the ones who forgot.
What reminder schedule works for home-service and med spa appointments?
Use the 3-day and 1-day texts as the base, then add a same-day touch where it fits the job:
| Appointment | When booked | Texts to send |
|---|---|---|
| Estimate visit (roof, HVAC replacement, remodel) | A week or more out | Confirmation at booking, 3 days before, 1 day before |
| Scheduled service call | 2 to 7 days out | Confirmation at booking, 1 day before, "on our way" text when the tech heads out |
| Same-day or next-day service call | Within 24 hours | Confirmation at booking, "on our way" text |
| Med spa treatment or consult | A week or more out | Confirmation at booking, 3 days before (with prep instructions), 1 day before |
Vendor guidance points the same way. Calendly's guide to reminders (June 25, 2024) says "it's acceptable to send at least two reminder messages (e.g., 24 hours and 30 minutes) before a meeting" and suggests a "combination of email and texts." Jobber's help article on assessment and visit reminders (updated October 22, 2026) uses a one-day-before reminder as its example. Both are vendor guidance, not research, but they match the trial: more than one reminder, with one landing the day before.
For home-service businesses, the 3-day text matters most for estimate visits, where the homeowner has to be home and often needs to line up a spouse or decision-maker. For med spas, the 3-day text is a natural place for prep instructions, so a client doesn't show up unable to be treated.
What should an appointment reminder text say?
Keep it short and include five things: who you are, what the appointment is, the date and time window, the address or arrival detail, and a clear ask to confirm or reschedule. Asking for a reply turns a reminder into a confirmation, and it tells you early when a slot is about to open up.
Sample texts (names, businesses and numbers are samples):
3 days before, estimate visit
Hi Dana, this is Peak Roofing (sample). Reminder: your free roof estimate is Thursday, 10am-12pm, at 42 Elm St. Reply YES to confirm or R to reschedule. Questions? (555) 010-0142.
1 day before, service call
Hi Marcus, Clearflow Plumbing (sample) here. Your water heater service is tomorrow, 1-3pm. Please make sure someone 18+ is home. Reply YES to confirm or call (555) 010-0187 to move it.
On our way
Hi Marcus, Jake from Clearflow Plumbing (sample) is on the way and should arrive in about 25 minutes.
3 days before, med spa
Hi Priya, this is Glow Aesthetics (sample). See you Friday at 2:30pm for your consultation and treatment. Please check the prep notes we emailed you. Reply C to confirm or R to reschedule. (555) 010-0163.
Things to avoid: long paragraphs, promotions tucked into a reminder, and a link as the only way to reschedule. Many people will just reply to the text, so make sure someone (or something) answers.
Do reminder texts actually reduce no-shows?
Yes, in the research that exists, which is mostly healthcare. A Cochrane systematic review published December 5, 2013 pooled seven randomized trials (5,841 participants) and found attendance was 78.6% with text reminders versus 67.8% with no reminders. The reviewers concluded that "mobile phone text messaging reminders increase attendance at healthcare appointments compared to no reminders." Text reminders also "had a similar impact to phone call reminders," which matters because texts cost far less staff time than calls.
That review is more than a decade old, and the authors rated the evidence moderate quality. They also noted that future studies "should report message content and timing in relation to the appointment," which is why the 2018 Kaiser Permanente trial above is the better guide to timing.
Do you need consent to send appointment reminder texts?
You need the customer's phone number and a reason to believe they want texts about their appointment, and you must honor opt-outs. Rules are different for reminders and marketing:
- Reminders and confirmations are transactional. They are about an appointment the customer asked for. Holland & Knight (May 27, 2026) notes the TCPA "permits certain transactional or informational messages without prior express consent, whereas CTIA guidelines expect documented consent for virtually all commercial messaging programs." In practice: tell customers at booking that you'll text them about the appointment.
- Marketing texts (promotions, seasonal tune-up offers, "come back" messages) are a different category with stricter consent rules. Keep them out of your reminders. Our post on AI voice calls and the TCPA covers consent for marketing outreach.
- Opt-outs: anyone who replies STOP should stop getting texts. Business texting in the US also requires A2P 10DLC registration; see what 10DLC registration costs.

How does Sharrk send appointment reminders?
Sharrk's AI voice and text agents book appointments onto the Sharrk calendar, Calendly, Cal.com or ServiceTitan, and text agents can book by text too. When a job is booked on the Sharrk calendar, the customer gets a confirmation email and text and you get an owner alert, with an overlap guard so two jobs don't land in the same slot. Before the appointment, Sharrk automatically sends the customer a reminder email and text, 24 hours before by default. If an appointment is rescheduled, its reminder resets for the new time. Every text includes "Reply STOP to stop or HELP for help."
Bookings are only half of it. Sharrk answers every call 24/7 and replies to new leads in under 30 seconds by text, call or email, so more estimate requests become booked visits in the first place. Leads from Angi, Thumbtack, Networx, Jobber, ServiceTitan, web forms and custom webhooks can each start a Strike, Sharrk's automatic follow-up sequence of AI calls, AI text conversations, texts and emails.
Mako, Sharrk's AI assistant, builds all of this from plain English. Tell Mako "create a 60-minute Roof Estimate appointment type, weekdays 8 to 5 Eastern, with a 30-minute buffer, and let my agent book it," and it drafts the appointment type and the agent setup. Mako can also build automations that start when a booking is created. Every change shows on an Apply card, Mako waits for your OK, and you can Undo.
You can build and test everything on Sharrk Free with 2,500 one-time credits and no card, then go live on Core at $99 a month. Want to see it first? Schedule a demo. See how it fits a med spa on our AI receptionist for med spas page.

Frequently asked questions
When should you send an appointment reminder text?
Send one about 3 days before and another 1 day before. A 2018 randomized trial of 54,066 Kaiser Permanente Colorado visits found two reminders (3 days and 1 day before) had a 4.4% missed-appointment rate, versus 5.3% for one reminder the day before and 5.8% for one reminder 3 days before.
If I can only send one reminder, when should it go out?
Send it 1 day before. In the 2018 Kaiser Permanente trial, a single day-before reminder had a slightly lower missed-appointment rate (5.3%) than a single reminder 3 days before (5.8%).
Do appointment reminder texts reduce no-shows?
Yes, in the research available, which is mostly healthcare. A 2013 Cochrane review of seven randomized trials found attendance of 78.6% with text reminders versus 67.8% with no reminders, and texts worked about as well as phone call reminders.
What should an appointment reminder text include?
Include your business name, the appointment type, the date and time window, the address or arrival detail, and a clear way to confirm or reschedule. Keep it to two or three short sentences and leave promotions out.
Does Sharrk send appointment reminders automatically?
Yes, for appointments on the Sharrk calendar. The customer gets a confirmation email and text at booking and a reminder email and text 24 hours before by default, and a rescheduled appointment gets a fresh reminder for its new time.
